I have just forwarded the ports, and that will do the trick. The easiest option is just to enable UPnP as that should be fine, however, older routers are not so hot on UPnP so port forwarding is the best idea.

uPnP also lets my flatmates file share like a mofo during the daytime, completely killing the net in the process even on a 20Mb line.

Just a note for others, if you don't want to use the DMZ you will need these ports open too:

Xbox Game Server Port (TCP): 22042
Voice Chat Port (TCP/UDP): 22043-22050
Peer Ping Port (UDP): 13139
Peer Query Port (UDP): 6500

I couldn't get any Voice communication to work without these ports open.

Even Microsoft themselves only mention TCP/UDP 3074 and UDP 88.

I got hold of daemon for my linux router called miniupnpd that has now set my status to open, rather than add the ports manually.
