I'm looking for some new pads for the first time in 3years (yeah down to the metal)
Do I have to buy the set with the heatsink on it or can I buy the cheaper set and re use the existing heatsink?

Can't reuse the existing heatsink, so you'll need the full ones, or get the uberbike equivalent where the heatstink and pad are separate.

I've used them for years, had few people moan about longevity but I've found they've worn well across mtb, road and cx bikes and as you say about 1/3rd the cost.

I've used Uberbike pads before, perfectly good.
As for the heatsinks... Like Begbie says, unless your banging out laps of morzine or such, the heatsink is nothing more than a pretty ornament. If a set of pads are lasting your three years, you are clearly not the type that sits on their anchors cooking your pads run after run, dont worry about the heatsinks.
