Gonna have to dig a bit deeper on this one after work tomorrow I think. But if anyone has any bright ideas in the meantime besides 'your drive is screwed', let me know!

Verify disk > repair disk

Restart, hold down the alt key, run disk utility from the recovery partition (assuming Lion, ML or Mavericks). Repair disk and permissions there.
