Signature form to wordpress site

I would like to add some forms to a wordpress website. I need around 5 different forms. Each needs to have a bit of text and then fields for Name, Address, Date and then a canvas signature (typed is not acceptable).

It would be good if the forms were emailed to me as PDF/jpeg, rather than having to check the website.

Can anyone recommend a decent plugin?

I only use 2 form builder plugins. Contact Form 7 or Ninja Forms. Both of which do not come with a canvas option, however a quick Google proves that 3rd party addons to these plugins can make that a reality.

That said, both will send you an email in HTML format on submit. I have not seen an add on to fulfill this requirement.
