for me the issue was apparently aero - turned it off after having tried a number of things (underclocking, drivers only (no CCC), etc) and not had an issue since. CCC is back on, the card is overclocked and both screens are at different resolutions and rates.
i got the crashes whilst running single and dual monitors before, with the crashes being far more frequent when trying to run the second screen. not even a hint of a problem since with aero off, even if it doesn't look as nice.
the fact ati and nvidia both have this same issue is a bit odd, and worrying given that neither have a solution.
