How do you organise your photographs?

I just use Aperture for the majority.

Processed pics churned out by aperture are just saved under 'Pictures -> Subject name'

Lightroom changed the way I deal with photos, I now have everything from the last few years in one place with the dated directory structure; it deals with all the photos from all the digital cameras I have had and keeps them in one place and enables me to manipulate RAW files and set up slideshows. Picassa screwed up my directories so badly it was only when I started using ALR which detects duplicate images that I was able to sort everything out properly, my photo directory and my lightroom backups are not copied nightly on to another drive to keep them safe :)

Correct me if i am wrong. But i hate how lightroom can’t pick up a sub folder.

Say I have a folder within say "Rally 2009" with the edited photos. It doesn’t pick this up and just adds them into the one library
 
Using Adobe Lightroom 2

F:/Photography raws/"landscapes, portraits, artisitic, unsorted"/"unsorted"date & name of shoot/001.cr2

1, go through them once to pick out the terrible ones and delete
2, move them into the correct catagory
3, go through them again and star them 3 - 5 based on quality/impact. 1 - 2 based on bad but wants to keep them for other reasons
4, process and burn full res tiffs to a sister folder structure identical to the raw one but only contains print ready tiffs.

as for renaming them.. probably best as when you get another camera you could end up overwriting file names. Its how I have lost so much valuable work.
I haven't got into key wording yet as I am still in an ongoing battle to perfect up the catalogue.
