Tax refund do I need to declare sorn?

I have car which is taxed it died last month so I applied for a road tax refund via here

How do you read the form:
-you need to fill in a sperate form to declare sorn as well as the refund?
-or would you read it that you are declaring it all in one with the refund?

Confused I thought it would be all in one, just got a letter back from the DVLA saying that havent declared it as SORN?
they kept the tax disk i sent for security reasons :rolleyes:

Is it just me or is it really misleading?
 
Isn't there a box on the form to mark to say you want to SORN the vehicle when you send off for the refund?

Edit:

Section 3 on the form:

"To declare that you are keeping the vehicle off the public road and you are the registered keeper, put ‘X’ in the box below:"
 
If it died, does that mean you took it to the scrappers? Because if so, they should've filled in the section saying the car's gone to car heaven and sent that off to the DVLA.
 
I have it in my folks garage atm , pulling some things off it then scrapping it.
