If the option is there, I always pick the one between normal and hardest. Let's face it, how many of us actually die or fail even once on normal settings? Sure we are mildly challenged but we never actually screw up. I've noticed this with pretty much everyone else I know, and every game I play on.

Maybe it's just me then by the looks of it. Minor majority of us anyway.
Main reason I do it is I rarely play through a game again unless it has a serious case of being different each time. Such is the likes of Oblivion for example. Or Halo as I love the campaign. But something like Singularity I will only play through once. So I try it on the hardest setting to 1) prolong the life of the game 2) make it feel that much more of a challenge 3) achievements!!!!! Love completing a game that has different difficulty ach's, at the end, getting 350 odd g for unlocking all difficulty settings is rather satisfying.
One thing I hate, is when settings are locked. Like Alan wake (I think) has a locked difficulty that you have to play through on hard to unlock the insane setting. Not sure it's alan wake im thinking of now... But you get what I mean.

The worst ones are those you have to play normal to unlock hard, and then hard to unlock the one above thatLuckily not many of that!
Max Payne comes to mind. That was a pretty dumb decision on the developers' part. The second game was even worse in that particular game modes which were awesome were completely locked off until you beat the main game itself. Dead Man Walking was the direct inspiration for today's firefight and horde modes we all know and love today.

I always play on the hardest level possible, as no matter how good the game is, I cant be bothered to play it all over again on another difficulty level.

LOL - Try Dragon Age: Origins on Hard - I'm playing on normal and have to leave it and come back again and again. Stuck on a bit at the beginning at the moment but having trouble with the party AI as they seem intent on getting themselves killed all the time
- I can only imagine how tough it is on hard.

I always play on the easiest the first time. I like to enjoy the story free from any irritation or having to redo bits. Then if the game is good enough I'll play it again on higher levels.

Always hardest. Naffs me off big time when completing it on hard opens up a 'nightmare' setting. FFFUUUU....
