theres no chance of sleep with 'proper' toothache
well unless it makes you pass out
your doctor can give you better painkillers and if its an infection causing your pain the doctor/dentist can give you antibiotics
Cold milk keep it in your mouth till it gets warm, always works for me but only last 20 mins then you got to do it again.

Codeine based painkillers are the only ones that relieve me from dental pain. Having said that I noticed how many people already said codeine does nothing for them. Everyone is different and reacts differently to substances. Hence why this sort of talk is banned here...
When I was waiting to have a couple of molars removed a few years back (had to wait for a week or so for an opening with the anaesthetist, and this was in a dental hospital too) he recommended I let an effervescent aspirin dissolve in my mouth next to the problem tooth - was the only thing that would touch it fast enough seeing that I wasn't in constant pain, the molars were too large and had lifted the back one out of the jaw, randomly exposing a nerve which wasn't the most pleasant experience I've had.
Unconventional but it worked for me, wouldn't recommend anyone does it without being told to do so or that is safe for them to try it by a medical professional, just recounting my own experience.
Seriously, if your suffering anything like I did back then you have my sympathy - my partner hadn't long had our first kid then and was still riding the no pain worse than giving birth pony, until she saw the pain that tooth put me in for about 2 weeks.
