Why is it that even when shave my head all over with the same grade I still end up thinner on top?

Your hair probably doesnt have a consistent thickness all the way round

You can generally get the shaver closer on top due to the shape of your head.

Nah a mach 3 razor does it nicely, use a trimmer and take it down as low as possible then just razor blade it, though the missus doesent like it cos she says i end up looking like a thug

I had that problem so just kept on going a grade shorter every time. It's now grade one and looks fine
