The sheer lack of haste in any multiplayer game when reloading your weapon. Yes, granted you want to make sure you do it right, but sometimes it's infuriating when you're hiding behind a small crate under massive fire and your character decides to slowly remove the magazine, tuck it away in his pocket, get out a new one making sure there's no dust on it, gently slide it in to the gun, wipe off the thumb print he just left on the side, **** the weapon, and then SLLLLLLLLLOWLY return it to position!

Another one; games that require you to put away your primary weapon to throw a grenade. Mainly in older games like Ghost Recon, but Crysis 2 was also a culprit of this! Here you are, in a mad rush to take out some bad guys and:
"Right, best put this machine gun away, I won't be needing this for 10 seconds! Okay, here's my grenade, best take it off my belt. Right, finger goes in here.... Give it a sharp tug; that's it - primed! Okay, I think I'll throw it over there, HFFFT! WOW, WHAT A GOOD EXPLOSION! Did you guys see that explosion? It was brilliant, so orange and firery! Oh wait, it didn't kill him. Dammit, time to gently place this ring on the floor and slowly grab my--... dammit, I've got another grenade out! Best select my machine gun again so I can fire back........."
