If...

Hmm, a big spray wouldn't be great.

What if the gun's muzzle was right next to the shootee to make sure the blood & gus didn't have time to spray? Could the contents wound him for real?

Wadding is used in shotgun shells to hold the gun powder in. Same deal with blank bullets which is why, or rather how, Brandon Lee was killed by a blank firing gun. The wadding was propelled out of the gun in the same way a bullet would have been and cracked his skull, causing a shard of bone to be fired into his brain. So if you're trying to fake a death with a blank of some variety, assuming that the "victim" is within range, you may end up with an actual death.

There's also the issue of the victim receiving burns. I don't know how far you have to be from a gun before the gases expelled pose no risk but again, if the "victim" is in range, they could be left screaming in agony from burns. It would kind of give the game away.

'blanks' are still very dangerous
Very true, i own a few shotguns. Ive stripped rounds out to test burn speed of various nitro celloulse powders. Those little flakes, (or grains if winchester) are extremely powerful even without a bullet/shot in front of them.

...you took all of the shot out of a shotgun cartidge and packed it with animal blood and flesh, could you then shoot it at someone and make it appear to be a real shooting?

Would it hurt the shootee?

I thought of it as a way of faking a death in a script I'm working on and thought it was fairly original, so I won't actually be doing this! Besides, I lack the gun, the cartridge, the blood & guts and the motivation. :D

squibs strapped to the person are easier, and far less risky.

you'd kill them, or cause sever injuries and burns setting of a blank let alone a live shell with the shot replaced with some other mass right next to a person.

Iv got some shotgun carts in the draw, all I need is some blood

Don't do it though. Very dangerous

Even if it did work it still wouldn't look realistic as the victims clothing wouldn't rip. It would look like the early bond films and westerns.

I've seen pretty realistic gun shot effects done by people using after effects. Pretty amazing what can be achieved for no money, as good as some multi million $ films.
