Yes, I would pay the extra. I'd say the pluses are that it's MSI, and as you say cooler and quieter.

*I'm not sure if this is correct but I have read that the Twin Frozr 2 is even quieter?* Results will vary of course. It's also cheaper that the Frozr 3 which might be worth considering.

I wouldn't buy it for £219.99. I think your best bet would be to wait and see if they will put it on the this week only sales or today only sales.

Wait until Wednesday and if it's not on sale, then just buy it anyway. TheMSI would be my choice. There are also some Sapphire cards that are pretty good.

I would just buy the stock card tbh, unless the one with the fancy cooler is the same price or cheaper. It is absolutely silent when in Windows and in games it really isn't loud (especially compared to the previous generations of ATI cards!).
 
Tbh i hardly hear the fan on my reference card. It sits at 24% at idle which you dont even hear, and during long gaming sessions the highest it goes for me is around 35%, which you hear, but easily bearable, temps dont go above 75%, all good.
