What's the best way of approaching this at an existing employer?

I've got another job offer on the table that doesn't match my existing package. I've found myself quite comfortable negotiating this as I've got nothing really to lose, however, I can't see how I would approach the issue when you've been at an employer for several years? I also see the fact that I have another offer on the table as potential leverage for a small pay bump in my current role.

Anyone got any success stories?

Yup, just bring it up with your line manager. Any decent company won't hold it against you. Business is business at the end of the day and staff leaving is a risk they should have accounted for, if your a member of staff that they don't want to lose and can justify a payrise to keep you it's a non-issue. Also depends on the pay structure at your work, if you have staff on fixed grades/incremental salaries there's not much scope for payrises.

Are you permanent or a contractor?

I would say something along the lines of - "I've been at the company for x years now and believe I am a more valuable member of the team and wish to renegoiate my renumeration"

Personally I've only ever negotiated a salary rise on the back of a letter of notice. I've found it's the only thing that will make them sit up and pay attention. I also accompany it with a speech about how I enjoy working for company x, but feel I'm worth more to the market than I'm receiving.

After my pay review and bonus last year I complained to my line manager that my pay after the raise was not in line with current market rates and that I was disappointed that I was being undervalued at the company.

His response was that he would get HR to do a salary review on my position, the result was a further 10% pay raise (I was asking for closer to 20%)

This was without another job offer or any veiled threats of leaving the company I just stated the facts to him.

Other job sounds like a reasonable approach. I'm not accepting a perm position from temp and they offer me more money. Oh well, keep it :D

Think though,. what happens when they turn around and say "Ok...cya" :p

If you want to be clever, a few days before you ask for a raise take the morning off work then turn up later in a smart suit. When asked, tell everyone you had a "meeting" - make sure your boss sees you ;)
