strange bald patch

I am sitting next to a guy must be about 21 , he has a really thick head of hair but with quite a big bald patch in the middle - really bald . Theres no scarring or anything - can this be natural ?

My mate did this when he was drunk. Went into a hairdresser and specifically asked for a big bald patch on the top of his head.
