Should a Smith machine be bolted to the floor?

My mate was doing an unassisted, seated, military press @140kg, when on the fourth rep the smith tipped forward, luckily and somewhat amazingly (to me) falling back in to place. I've been in to a few gyms and noticed the Smith has never been bolted to the floor or stabalized in any way. Has anybody else ever noticed if their Smith was fastened/secured? Thanks.
 
I know the one at my gym isn't. Nor is the squat rack, always scares me incase I have to dump the weight!

Not my problem if it falls over and damages something, but if it did it would take out the person on the bench!

Yeah he's a massive bloke. I mentioned him before, five foot 10 (or thereabouts) 22 stone. He was pressing the 140kg whilst recovering from a slight injury. His normal is 6x2+. I was standing behind him but when that Smith tipped I just did not want to be anywhere near it. He didn't tell me this until after, but he normally has two blokes standing on/across the frame when pressing.
 
I had a word with the manager about it and all he could say is he would have a word with my mate (as if he was using too much weight)?
