Does anyone know where I might get the following (and 2 other similar sized and complexity) piece made? And any idea how much it would cost?

I'm thinking 2mm steel would be suitable.
 
Folded would be fine

The big square holes are more than big enough to fit over some other stuff so the tolerances on those are much wider, say +/- 2mm or so.

I imagine you may have some accuracy issues with folding a 2mm piece in that exact location, due to the location of the hole. However the rest of it looks fairly simple - +/-2mm is a fairly wide tolerance, I'd be asking for less than 1mm.

The hole wouldn't have to be flush to the bend, it could be up to 3mm away
 
How do I do a more accurate drawing of something that doesn't need to be as accurate as the one I have done?

Well, for example, you say that the bend could be up to 3mm away from the large hole, so you need to define how far away it is, rather than saying "oh, it'll be close enough".
 
You may be right, but most normal people would get the gist :P It's not something that would be CNCd.

It looks to me as though the hole on the upright section is flush with the fold; if you send that to a company to be manufactured they'd assume that's what you meant. Given that pretty much every other feature on the drawing has dimensions, why wouldn't you add a dimension to that part, if it could be up to 3mm away?
 
Just a piece of advice, if there is any load on this motor at all you will want to put a couple of triangular braces at the point where the bracket is bent or you will get flex when the motor starts and stops

Just be careful when folding that close to the hole. It'll pull and give the hole a nasty lip. I can't see the pic (on my phone) but perhaps drill the hole after folding :)

Edit: just see they're square holes not round. If you put a relief slot 2mm deep either side of the fold near the square hole it should stop the pulling.
