Wearing a helmet wont save your life if you get crushed by an HGV. However, if you clip kerb or something similair, it'll stop brain damage from you smashing your head into a railing, car, road etc.
My brothers mate was cycling the Honnister pass in the lakes, overcooked it on a wet corner and went straight into a stone wall, head first, and the wall didn't budge. His helmet was smashed to bits, his head suffered a few injuries, but if he wasn't wearing his helmet he'd be dead.
As for the straps, yes they should be tight enough so that if you do come off, it's not going to flap around on your head and completely miss the impact area.
Quite how some people think that helmets are useless/you can wear them loose is beyond me.
