Cat advice - help me with my psycho kitty!

He isn't neutered no
Lovely I'm assuming he isn't a house cat so he is procreating with every female cat he can get his paws on. Please be a responsible owner and get him castrated so there isn't unwanted kittens walking around the streets!

Also make him do his business outside, haven't ever owned a cat who I let do their stuff inside the house, also let them outside.

I'm no expert, but something is telling me that crapping in his litter tray would help the situation some how. Report back.
My cat has the odd 'turn' about once every two weeks, he will just attack us when we're lying down. Found the quickest way to stop him is to pick him up and hold him in my arms.

Poor cat is going insane with mixed signals.
He is marking his territory with his scent. His litter box is a huge sign saying "this is my place". I would bet a weeks wages he is also "spraying" other areas of the house and also rubbing his his cheeks on everything including his human pets. The cheeks have an area of scent glands too.
Be responsible and kind. Get his nuts removed. He will be a lot happier, more loving and won't stink your house out by marking territory.
Not to mention he also won't be responsible for hundreds of unwanted kittens.

Is this Sam your having issues with Op?
if it is, and thinking about the issues you had with him when you first adopted him, then he is defo using his litter tray as part of his territory (as mentioned in this tread)
Try just removing the solids from the tray first. Only change the whole tray when needed. What litter are you using?
TBH giving him the snip will also help with any attitude problems he might have.
Did you know that they take all of a cats tackle when a male cat is neutered? Apparently if he still has the 'meat' (ie: meat and 2 veg) then he still might try to rape any females he sees...
