It appears rude to the greater population so whether designed to help the traffic flow or not it doesn't have that end result when the larger number of people aren't using the system properly.
Doesn't help that the road marking and signage are often ambiguous at best - one of the biggest problems I see of it is on the A303 heading west just before stonehenge the road signs and markings even appear to encourage forming a one line queue about 400-600 yards before the 2 lanes actually merge then at the actual point of merging they are vague at best.
The last ~800 yards should have completely different road line style and signage to actively encourage people to use both lanes when necessary.
Still a lot of people on the roads, though lessening for obvious reasons, who learnt to drive when thing were very different, without a wholesale approach to adjusting people's attitudes to it and bringing it to people's attention who may have never even been taught about it or given it a second thought nothing helpful comes of anything, people can gripe about how it "should" be done however correct they may be but that doesn't really achieve anything.

Never seen a sign saying to use both lanes...
It's always the ||T or |TT, accompanied by instructions to Get In Lane, usually on the left. That's when you get the loonies hooning down the outside in theri BMWs and Range Rovers... until they meet a truck or two holding the line and keeping things going, of course!!

I've seen it in 1-2 places, mostly just off big roundabouts in London and Bristol, etc. but most places the signs are vague at best and counter to proper use at worst and often the markings and layout tend to be bias towards merging into a single file of traffic. (possible even due to lack of understanding by the people working on the layout).
Its not really something thats ever going to be resolved without bringing it properly to people's attention and adjusting the roads to encourage proper use.
There's never any signs about that say 'don't throw poop' but it doesn't mean you can throw poop at everyone.
If you need a sign to tell you to use both lanes, god help us all.

That's my point - Those ones I mentioned are telling you NOT to use the closed lane, *because* it's closed.
I'll generally try to merge a bit before the cones mainly due to prior experience of driving casually down the still open lane and then no ****** will let you pull in... some even having a go :O it is silly as if they just moved slowly with good distance they could merge nicely but nope... they'd rather sit on the bumper of the car infront and stationary 
also we do it quite well here in swansea xD there two single carriageway roads that merge to make a dual lane, one way system. right at the start of this system where the right lane joins after a 90 degree turn they have about 30ft of bollards (guessing some accidents from people changing lanes too fast ) but gives you a few moments to look over and assess the other traffic. Basically almost every car in the left wants to be in the right and all the right traffic want to move to the left. get this every morning and to be fair I've rarely seen any issues, just the rare pillock who is determined to throttle it to close that gap so you have to go in behind them.....even then most of the time they change lanes shortly after xD

I tend to merge in early, purely because if I go past the first lane of stationary traffic, I often find the people in that lane then won't give me room to merge and will try and close the gap as tight as they can.
It's when they realise they're being monumental clowns and don't even acknowledge they've done it by looking at you/your general direction and just pretend they haven't seen you that makes me laugh.

I've seen some that say "Stop here when red light shows", if that counts?!
It doesn't, because it's indicating the position to stop in the absence of a formal stop line, not the obvious 'if its red that means don't go, duh'

Yes, but you're still supposed to merge at the point of closure, not 1/2 mile before and then get butthurt when people who are following the highway code correctly overtake you in the clear lane.

I'd have done it on my bike, and I'd have done it if i was in a car. Don't see the problem.
