Bosch sds adapter drill bit help

Hi all. So I've got a Bosch SDS Plus adapter for my drill and this is now my third one! I keep going through these mainly because I don't understand how to put the adapter together. The instructions on the box tell me to first screw in the holder underneath the adapter and then the screw in through the top of the adapter. However every time I try doing this the screw doesn't go in properly at all ie not catching the thread lines. Any help? Below is a link of the adapter in question.

The shank screws in clockwise, the screw anti-clockwise.
