Horrendous TITAN Power throttling (+crashes)

Witcher 3 was crashing loads after I got about 15 hours into the new expansion. The game would either freeze, followed by a loud sound loop then a black screen or the screen would go black, sounds would continue normally for a few seconds then I could hear lots of windows errors and device disconnecting and connecting sounds.

I reset my CPU overclock and it didn't help. I reset my GPU overclock and it didn't help. I reinstalled windows 10 and it didn't help.

I flashed the original bios back onto my card and it seems to have reduced the frequency of the crashing. However it has horrible power limit throttling now. The base/boost of the card is 837/876 yet all games (W3, GTA V, CSGO, Unigine Valley etc) run at <700 with power hovering around 100% and power limit=1 in afterburner. I've reseated both the card and the cables but nothing seems to have helped.

Too much OC,try lower it.The vga stuck on low3D state,you have to restart your pc.
 
i had a similar issue once, drove me nuts, where some games froze with sound loop or kept going on black screen...turned out to be faulty Ram.
you could do a memory test or install the Ram one by one and test if there is faulty one.
 
Thanks for the advice so far. I hope it is the RAM as it's a much cheaper fix.

Running Memtest86 and it has found errors.

Seems the issue was a loose power connection somewhere. Removed the Cablemod cables and everything is fine again. Put them back on and it is still fine.

thats what i thought too, because after days of scratching my head, i noticed that when i unpluged everything and pluged it back in, the issue got solved, but it starts all over again after couple days, also when i increased voltage of the ram the issue became less frequent, i only got rid of the problem completely when i changed the ram.
i hope it doesnt show up again for you, but just letting you know if it does happen again :D gl
