I currently have an Asus TUF B450-Plus which only had 2 full size PCI-e slots, the rest (3) are just PCI-e 1x slots, are there any B450 boards that have more than that or do I have to step up to the next level to get what I'm after?
I run an unraid server off an AM4 platform and need more slots for expansion.I don't think so - even if the slots are mechanically x16 slots only the first is electrical x16 gen 3, with the second being x4 gen 2

You probably want to be looking at x470 or X570 if you want more (but still not enough) lanes
Threadripper can meet your needs no problem - but it's not AM4 obviously

Where would I mount the card though as the board is ATX and my power supply sits at the bottom (Define R5).I've got that Asus board in my server for the exact same reason, couple of RAID cards, network cards etc

I use a Silverstone ECS04 for 8 extra sata ports on my server with x470 motherboard. Potentially you could fit 2 of a card like that on x470 and then use some thing like a Zotac GT 710 which is available in PCIe x1 to drive a screen if you don't want to run headless
