The problem is that lessons very rarely are actually learnedTo be fair "lessons need to be learned" is a valid statement.
When learning is not applied to force change, that's when there is a problem.

IIRC the aviation industry takes "lessons to be learned" extremely seriously, which is why after pretty much any major incident there are changes and improvements as they look at all the areas where things went wrong making changes to reduces the chances of it happening again (even when that costs money), rather than just finding a fall guy/saying "things have changed".
And the whole industry across the country/world takes on those lessons.
