I'd try a different driver or two first.

Its done it over the past few driver updates havent gone back to older ones to see if that fixes things

You also get an option trough the driver install under advanced to do a clean driver install.
I tend to use this at work when remotely re installing or updating Nvidia drivers.
As mentioned above, go back to an earlier driver or the last 1 you didn't have the issue and see if the problem is still there.

is that enough to warm the card up to the point where silent mode ends and then fans come on and once it hits 30c they turn of till it slowly warms up again?
5090FE owner here too. At idle/ on the desktop - The fans kicking in at ~1100rpm on a cycle and then going back down to 0rpm is considered 'normal' as far as I'm aware.....
I've tested it and it's due to the card warming up to around 51 degrees when fan stop is on (so fans at 0rpm under a certain temp at idle). However, at idle at 51 degrees the fans kick in at 1100rpm and when GPU temp goes back down to 38 degrees the fans stop. GPU warms up again, and the cycle repeats.
Mine has done this since day one and I looked it up at the time and other FE owners were seeing this behaviour. It's barely audible at 1100rpm but the change from 0 to 1100rpm and back down every 10 mins or so is a bit annoying if you value total silent PC running.
Is this the behaviour you mean???
You could stop the change being annoying by using Afterburner and setting the fans to always be on at idle, but I don't bother.
However, also at idle, one of my fans will also very randomly kick in at full power 2000+ rpm for about 10 seconds and then go back to 0. Sounds like a jet engine. No idea why it does this and it's rare and random, but I'm assuming it's the usual Nvidia horrific drivers that the 50 series has been plagued with since day one. Again, this only happens at idle, never in gaming.
