I got a code and just entered it.
Here is the link to the page, you should just be able to add it to the basket as normal as you will have the MW:R token, I assume you have paid for your order?

I also have to say this game is fantastic, Raven has nailed it 100%, I am so hyped for the multiplayer now.
The game plays so well, it's classic Call of Duty, none of this acrade action game that the series has become, just straight up greatness, it takes some getting used to because of the conditioning that the series has now instilled in players of being a fast paced first person action game, but I've been slowly adjusting with the few hours I played last night.

I started on normal and the accuracy of the enemy's on some stages is ridiculous, can't even begin to imagine the vet play through!
I think I only got about to level 8/9 all those years again on vet. I remember raging a lot and it just turning into a crawl to the checkpoint grind.

The lobby screen is just as amazing as the game.
I was as green as they come when the game was originally out so I have no idea about the OP of the things people complain about but I didn't see it myself last night, I suspect those that had problems with it back then will now because I don't think anything has changed at all with how things work.
