I currently have a :

512MB MSI RX3870-T2D512E-OC

And whilst its a great card, its so darn noisy!! The usual ATI fan tweaker program (cant remember its name) will not reliably reduce the speed either.

What are my best options? Are the fans on these easy to replace with somethign quieter? Or is the speed hard coded so it would make no difference?

I wouldn't mind buying a new card possibly, as long as it is at least as good (ati or nvidia, I dont mind), and as quiet as possible.

You can get a few 3rd party heatsinks that'll silence your card :) As for new cards, the GTX260s are about as fast as you can get if you don't want load noise.
 
Have you used rivatuner?

Aha! Thats the name of the tweaker program that doesn't reliably reduce the fan speed - it resets to default almost every time you open or close a program :(

Accelero S1 rev 2 would work well if it fits that card

That looks like just the ticket!
 
Did you create a custom fan profile? I've used rivatuner and using that it always stayed at 55% fan speed.
 
have a look at thermalright coolers. Don't hold me to it, but I think they made a HR-03 version for cooling HS3870's. I've got one on my 8800GTX and it dropped 30 degrees with a 120mm on it which you can't hear, or 20 degrees drop without a fan...
