Does a gaming mouse really make a difference?

It does. Going back to my awful Dell mouse in work everyday is such a downgrade. In every regard. I have a wireless Razer Mamba at home and everything is so much better, obviously technically, and the polling rates etc. but even just the feel of it. It's night and day.

Bought a Roccat Kone XTD in the end - and am very happy so far, but not had a chance to use it in anger yet as such - so I will see how it goes.

This is horrible advice. It's completely incorrect.

Changing the Windows sensitivity slider is a bad idea and will result in skipped pixels.

High DPI =/= more accuracy. Every mouse performs differently at different DPIs. The Deathadder for example performs best in terms of perfect control speed and overall tracking quality at 1800 DPI despite being able to go much higher than that. Where do people even get the idea that DPI = accuracy? DPI is simply hardware sensitivity...and nobody actually thinks higher sensitivity = higher accuracy, because the opposite is true. Lower sensitivity means more room for error, one tiny wrist movement at high sensitivity throws your aim off completely. You have more control when you aim with your whole arm while using low sensitivity.

I recently bought a Logitech G700 and hate it. It replaced an old G5 mouse which I loved but finally died. Need to get something else cos I can't game with 6000 buttons;)
 
I use Razer Abyssus, most basic model in their range.
3 buttons, scroll. Thats it.
Sensitivity mode change, 3 modes.
Very comfy and works great for almost 2 years without fail.
I play some mmo and fps games and this mouse is perfect for the job.
I don't like the zilions of buttons - can't use them anyway, because of large hands.
