It almost looks like a setup but Im undecided, still funny though.

I can only imagine they just didn't put the nuts on at all even untightened nuts wouldn't all simultaneously fall off would they?

I'm sure the cause was the wrong sized bolts attaching the wheels. Which I have no idea how the person who put the wheels on did.
