Perhaps not as dramatic as the title sounds but...

OCUK motor geniuses, i need your help

My 08 Ford Focus has a very intermittent fault - 2 or 3 times a year, the display will show 'ENGINE SYSTEM FAILURE - REDUCED ACCELERATION'. It goes into limp mode.

My OBDII reader showed P2125 and P2138 errors

P2125 - "Throttle / Pedal Position Sensor / Switch "E" Circuit"
P2138 - "Throttle / Pedal Position Sensor / Switch "D" / "E" Voltage Correlation"

Usually, turning it off then on again resets the fault and no problems.

2 years ago, that didn't solve it, but the internet suggested a faulty battery was causing a low voltage error. Got the battery tested, it was faulty, got a new one and the error was gone.

Until today - had the fault all day, no amount of restarting will clear it. I've had the battery checked at a garage, no problems at all.

Any ideas? Could the sensor have actually gone, despite all previous issues being voltage related?

Thanks!

Could as easily be wiring - I'd be inclined to check the condition at the connection to the pedal, it's often exposed and can rub after a few years.

If that seems ok i'd do some more thorough wiring checks before assuming sensor

With stuff like this you'll just need to look around the pedal and and position sensor in the engine bay (probably near the throttle body) and see if there is anything obvious. Might be the one on the throttle body or the pedal.
