Excel form to add new lines to data sheet

Hi guys,

I need to set up a data entry form consisting of a number of options, including:

Age range - 4 options
Welsh speaker - yes or no
Ethnicity - 5 options

I can create the actual form with combo boxes etc, but these seem to only link to individual cells. How do I fill in the entire form and them throw this info over to a new row on my data sheet?
