No, you don't have to to refuse the sale. Proxy sale law doesn't apply to games, DVDs, etc. If a parent wants to buy a PEGI 18 game for their 12 year-old that's entirely up to them. If the retailer wants to set its own policy on the issue then that's up to them but there's no requirement to do so.

You are correct regarding PEGI ratings as they are only a guideline in the eyes of the law, however it does apply to BBFC rating which FC4 comes under along with many other 18 rated titles such as GTA5 and also any DVD which is rated greater than PG

Selling to a minor, yes. Selling to an adult who then hands that game to a minor is not.

Nope, retailers are legally required to abide by PEGI ratings as of a few years ago as they took over the role from the BBFC for game ratings.

By proxy applies to all age restriction recognised by UK law

There's no offence committed if someone buys a game to provide it to a minor, or if an employee sells a game to an adult while there is reason to believe it will be supplied to a minor.

I've been unfortunate enough to spend the last few years in retail and the sale of age restricted products is one thing they're pretty hot on. As long as the person buying the game is the right age, it doesn't matter who it's for.
 
Given the drug references/taking/acid trip missions, torture and general gore I'd say probably not for an 11 year old!

I think most children could handle the violence/gunplay of something more straight laced like COD but I think FarCry's casual satirical approach to drug taking and murder probably isn't appropriate for a child that young.
