Getting disk replaced by dev?

I've seen a lot of you talking about having disks knackered by xboxes and whatnot and sending them back for a free replacement disk. Anyone care to enlighten me on how you go about this and whether you have had success.

I believe it was only MS published games that you could do it with, by filling out a form from the MS website.
