I play a golfing mmo with a guy and this is how his screen looks while playing the game. He's reinstalled the game and updated his graphics drivers. Every other 3d game is fine as is the 2d side apparently. It's Vista but I'm not sure which flavour. Sorry, the details are scant.

Does anyone have any clues as to what may be causing this?

I don't know. Try updating sound drivers, chipset drivers for the motherboard, maybe a new bios and the latest graphic driver. If only one game produces this then it sounds like a conflict rather a hardware problem.
 
Seen my desktop do the same thing with an ATI GFX card and a big monitor, I had to delete the monitor in device manager, and let windows redetect the same flamin monitor. Then it all went back to normal.
