Hard Disk Problem

All over.
I've got a 300GB Samsung Spinpoint, used as a storage disk.

Recently I'm getting a whole host of errors regarding this disk.

An error was detected on device \Device\Harddisk2\D during a paging operation.

Event viewer is filled with thousands of the above entries.

Disk management says it's healthy. I've done a chkdsk and it crashed at stage 2.

I can still access it through windows, although it's very slow and freezes when I try and do anything with any of the files located in the hard disk

Has the disk failed?

Can anyone help please?
 
I ran that diagnostic, and it seems there are no errors with the disk, which is a relief. Must mean that it's got something to do with Windows or the S-ATA cable must be damaged.

If it's a Windows issue what do I do?
