I picked this up on 3 CDs in a local game shop and I'm aware it's particularly buggy in unpatched form and I'm sure there are loads of mods and fixes for it. What do you recommend I install for a first time play through?

Incredible game! But do not play it without 1 of the patches. IIRC there are 2 main ones. 1 made by the dev team in their own time (and unofficial, the game was released too early and they knew it) and another made by fans which has done a lot of work on the game.
Oh wow... Fabulously atmospheric game. Not played it in so long, going to have to play though it again.
The haunted hotel stage... Really creeped me the hell out.

That hotel level... I was playing it at night with surround sound on... My wireless mouse became one with a wall and I held the PC power button down and waited until daylight to even think about that again
