Help please with form to cgi email script

Hi im having problems getting the below form email script to connect to the contact.cgi script on my host cgi-bin, could someone look at it and tell me where im going wrong please, I do not understand cgi stuff.

The error I get when form submitted is:

Software error:
cdonts error at
\web\content\cgi-bin\contact.cgi line 45.
For help, please send mail to this site's webmaster, giving this error message and the time and date of the error.

By the way it is hosted on easy space and came with a iopost.pl file in the cgi-bin but I dont know how to use it.
