it looks like you attempted to install windows or something has been installed on disk 0
disk 1 and 2 are correct, you can click on the D:\ right click and click extend to use the extra 1678gb.

from what i can remember windows 10 asks if you want to create a recovery partition

It's because its installed to disk 2 and automatically put the system reserved onto disk 0.

Personally if not much were done i'd put the 250GB disk in sata port 0, unplug the other two drives and reinstall so you get this:
(or just unplug the two 4tb drives and reinstall, it doesn't really matter i just prefer to have my boot drive on sata 0 as disk 0 )

Then plug the other drives in and clean/format them as GPT.
The alternative is trying to move the system reserved onto disk 2 which is a bit more faffing around
