How do I make a comments page?

So as the thread title reads really.

Just something that I can easily design that allows vistors to leave their basic info such as name, email and an area for comments.

Easiest way to do it is to create a form in the HTML then use PHP to take the data and stick it in file or database and the display it on the page.

Loads of PHP info in the stickies.

Yes I did but please dont think im as web html literate as a lot of you are. I struggle with this stuff.
