How do you get a stable GPU OC?

I'm really scratching my head on how to achieve a stable overclock on my GPU. I can move the values up and run FurMark and Unigine 3.0 for hours with higher clocks and volts and it never crashes(it does obv but I lower clock or add volts for stability)

When I find it to be stable in those two benches above I begin to play games, but when I play games like starcraft2, modern warfare 3 and quantum conundrum the drivers crash/freeze pretty soonish.

Anyone have some advice on ways of making my gpu stable with higher clocks?
 
personally i use the games that I play most as my stability test alongside with heaven that way Im sure that it wont crash during gameplay

This, I dont use furmark as it totally cooks cards. Im sure many agree to. I just run Heaven for about 30 mins, if it makes it, happy days, if not i back it off. Ive never crashed in game on a clock that passed in a heaven run.
 
OCCT 4.0, easily the best OC testing tool, doesn't seem very reliable but Crysis crashes everytime the GPU core clock is even slightly too high.

In my experience raising the voltage did precious little. Even beyond the safety limit the GPU core might go 50mhz more than stock voltage and after that it was having none of it. I've used universal water cooling and extensive heatsinks over the VRMs, VRAM since 2006.
