How would you define this sentence?

"One of my favourite, if not favourite game of all time"

One of his favourite, perhaps his most favourite game of all time
One of his favourite, but not his favourite game of all time

Do people use if not differently, I think its the first one btw.

It is down to your inflection of the sentence. Can mean either.

That one, the original sentence doesn't really make much sense as text, its meaning would rely entirely on inflection.

This. You could say the second 'favourite' in such a way that it'll suggest you have a favourite favourite game that isn't the one that you're talking about.
