Online play can be in any mode, all depends on how the server has been set up, hyperlobby is the main online meeting point.

Things that can be set in the conf.ini that are not available via game setup menus are widescreen aspect ratios, effects rendering, water detail levels, advanced cloud types and probably a few others I'm missing.

Go join the forums over at www.simhq.com and post questions on the IL2 forum over there, you're likely to get better information than relying on me
Grab the latest patch which is 4.08m available here:

As for mods/addons there are a fair few, although strictly speaking they are all addons as the game isn't moddable apart from in a very basic way. Plenty of campaigns and stuff for you to grab, links can be found over at the official site.
