Interactive Form / Wordpress?

Hi I'm looking for something similar to this form where you can tick boxes or enter criteria that then shows an appropriate output.. in this case which therapist is best matched

You could use JS and PHP to show the posts with the same tags as the check boxes ticked. Like a filter system. Seen it been done before, it is quite common.

Not great with server side code yet - could you show me any examples or give an example? I know what you mean just wouldnt know where to start myself
