The keeper was off his line when the pen taken but the ref didn't have the cajones to call it.

That doesn't mean you should overlook the rules. Look at the replay he was a metre forward of the line!
Plenty of other areas rules aren't enforced correctly that are more important though
Example - Wilshire conceding that penalty
I think if you stutter its near impossible for the keeper not to start shifting, you run the risk. Just take them like Gerrard or Lampard and its no problem

I literally don't see what Milner offers football.
Works hard and does a job. He's not a winger though, complete waste of time putting him out there.

I don't rate him in the middle either, but he is even worse on the wing. He seems to try hard but has no quality to back it up. He'll chase down a ball, but then not get a cross in. He had all day to get a cross in before when Baines (I think?) made the overlap and drew the defender away. What does he do? Crosses it straight to the keeper.
