Ipad app to collect customer data that isn't completely over the top

Hi all, im looking for an app, ideally for ipad to collect customer data at trade fayres/shows. It's stupidly hard to find apparently. I just want to get their name, phone, email etc to chase up leads for my business services.

So far all i can find is completely OTT apps aimed at massive companies or subscription based apps with a huge amount of other useless/complex stuff i don't want.

I've been using my own webpage contact form for now, but the internet failed at the last fayre and it lost the data the client filled in; then they understandably couldn't be bothered to do it again - so i need a simple form to collect data that works off line - any ideas how ?!?!

The only thing I can think of is Google Forms or Microsoft Forms but unfortunately neither of them solve the problem of the internet going down. If your iPad has mobile data you could have Wifi and mobile at the same time reducing the risk of no internet.

Could look at JotForms or MailerLite, they offer offline forms and both do a free plan although they may be a bit limiting. Otherwise there are other form capture platforms like Gecko, QuickTapSurvey but most are subscription based.
