Is my SSD dead?

I'm running a Corsair F120 and I've just got my new build up and running. It had a install of windows from the previous machine so i reformatted it and installed a fresh version. But after that I just got weird restarts and the odd blue screen that flash up so quick i couldnt see the error.

So i reformatted (again).

I booted from disk and once the welcome screen appears i hit SHIFT + f10 and typed "diskpart"

then "list disk" - i then saw my SSD so i typed:

"select disk 0" then "clean"

I then reinstalled windows on the SSD and when it gets to the windows completing install part, where it restarts and it should then prepare for first use, it blue screens and then restarts - but again its too quick for me to see the errors.

Any ideas? I'm totally lost as to why that didnt work. Also the SATA mode is set to AHCI already. And i also noticed after it tried to reboot itselfa few times it then says there wa a problem, do you want to run windows as normal or in safe mode and if i choose normal I then get to the "setup is preparing for first use" - but again after a few minutes it blue screens and restarts. Back to square one. Driving me slightly mad now.

Is it possibly fubar? Ive noticed its visible in the bios but sometimes it does seem to vanish which again, i cant explain but might point towards something?

what motherboard?

i7 4790K (Devils Canyon)
Gigabyte z97x SOC Force
8GB Corsair Dominator DDR3
2x Saphhire 290s

What are the rest of the system specs, you could have a faulty memory module.

Try running men test to rule out any other issues:

Bootable ISO so you don't need a operating system installed.

Thanks, will give this a go tonight and let you know the results. There is also a sys fan header at the bottom of the mobo not working, although i figured it might just need a bios update to sort that, not sure if its related.
