ITX Case options - need small and great cooling.

Im looking to knock up a small ITX setup.
I want it to be as small as possible, however, I need to use a Q6600 in there and the cooling needs to be superb.

Any suggestions please?

Or even some basic advise on basic cooling?
I have always gone with ATX Full sized boards and I know a fair bit about cooling, but I know zilch when it comes to ITX? - I have 2 other ITX setups and one is in a small-ish box, but has full ATX PSU and a Mobile CPU so sooling does not have to be so precise, as it hardly runs anything requiring any power, my main Linux PC I knocked up a few weeks ago, is also ITX but an AMD A10 and thats also a full-on setup inside a Globalwin 130 so why I nwent ITX I dont know but its a wonderful setup, but this time I want to go small. This is something that I have never really done before.

Now, in saying this, I have just come across a silly small case, that will hook up to the back of a TV and its a basic PC that has died a death and I grabbed it and sure enough, its dead as a dodo, and so I have been twiddling about with the case, and I have done a basic mod of making a huge hole in the top/side of it, and mounted a 120mm fan onto the outside of it.

This will allow me to either have a bigger cooler sticking out, or preferably, to use a non-fan very flat cooler onto it and maybe that might help?

Other than that, I have seen a very small case that will allow me to have the ITX Board lay flat, but also next to it, allows a full sized ATX PSU, so, this is not completely ideal, it will look acceptable in that it is more or less the same shape as a typical DVD player or the sky box ( or that rough size ) and that may be a useable option.
