Sharp thin needle and Swann Morton scalpel both useful at times. Mostly though I'm lucky enough I don't tend to get deep splinters. Good soak if you're really struggling, then open up the entry point til you can grab the *******. Oh, sharp tweezers too I guess.
Needles and possibly tweezers. I do remember a white coloured cream that was once made up for me to aid with "drawing" out a particularly deep one. I can't remember what the cream was called.
I do remember getting one particular splinter that ran the full length under my thumb nail. It seemed to start about 2mm in and go right to the end. That one still makes certain bodily parts contract and clench just at the memories of getting it out.
EDIT: Magnesium Sulphate is the name of the cream.
