A disaster waiting to happen, what happened to foresight when designing such things?

The inventor of the clutch must be spinning in his grave with all these people crushed by gates and such like.
 
Reminds me of a scene in the film 'Signs' when someone is crushed against a tree following a car crash.
Poor guy. Shocking, just shocking.
 
Why on earth didn't the extender have some sort of sensor to detect something between the it and the train, or maybe a safety switch which would reverse the motor in such an event.

I bet they will all be checked now.
 
I am hoping a re-think of this design will happen after this accident, truly terrible design. Bloody Americans always go one step further.. surely a mind the gap audio announcement and a station guard works.

Hope he pulls through.. I can imagine his injuries are very severe.
 
Why are there pictures of it? I find that a bit disturbing cause if he doesn't pull threw then his family won't want those pictures to be around.

If he survives and wants them to be posted around then that would be fine as it would help to tell a tale, a tale we don't need visual aids for at the moment.
