Missing W10 start menu icons, ideas?

This is the third W10 install i have had this happen with, does anyone have any idea why this is happening?

Seems to be a reoccurring bug with Windows 10, I had it for a few weeks before, similarly with tiles going transparent as well.
I used to just restart the Windows Explorer process which resolved it temporarily.

The fix was it was eventually patched but guess it's cropped up again, unless it's an old version of W10?

Its fully updated. Restarting explorer does work so i guess the bug is back?

Funnily enough (or not)... I had this happen on the Start Menu tiles, I had to expand them to medium and then back to small for the icons to reset.

Very weird.
